Please arrive about 10 minutes early to fill out all required forms. If you would prefer, fill out our New Patient Form online before you arrive.

*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.

Initial Examination

When you visit our office for your first appointment, your dentist will begin by reviewing your medical and dental history and completing a comprehensive dental exam. Your exam will include a thorough examination of your gums, teeth, bite and jaw, an oral cancer screening and any required X-rays.

Treatment Plan

After a review of your history and examination results, your dentist will develop a personalized long-term dental treatment plan for you. Your dentist will also take the time to answer any questions you have about your dental health and treatment options.

Insurance & Payment Options

At St. Laurent Dental Centre, we'll help you navigate our options for payment, direct billing to insurance companies and financing so you can focus on getting the dental care you need.

  • Insurance Claim Submissions

    Our office does not accept third-party payments. Payment is required upfront from all our patients and the insurance company will reimburse you. We will still send the claim to your insurance directly on your behalf.

    We submit your insurance claim electronically, resulting in faster reimbursement, without the need for paper forms and signatures.

    Our administrative team can assist you in understanding your insurance plan better and can also send estimates to your insurance company prior to starting any major treatments. 

    Please bring your insurance plan information to your appointment.

  • Payment Methods

    Any co-pays or amounts not covered by insurance are your responsibility and must be paid at the time of your appointment.

    We accept payment by cash, debit or most major credit cards.
